Register for a Portal Account
To sign up for the Patient Portal let the staff know at your next clinic visit. They will send you a link to your email for you to set up your secure account.

Access Your Portal by Phone

  1. App download from your Apple or Android phone search for the HealtheLife App.
  2. Sign-in to your account (patient portal access info) > Enter your username and password > Sign-in

How do I reset my portal password?
To reset your portal password online, click the link below the patient sign-in on the Patient Portal Log-in Page and then follow the steps.

Can I be the proxy for my child’s Patient Portal?
Yes, parents or legal guardians may act as a proxy for a child under the age of 12. Patients between the ages of 12-17 will be able to access their own portal and may choose to grant their parent(s) access.

Can I be the proxy for another adult’s Patient Portal?
To be the patient’s proxy, you may visit “Release of lnformation” in the Health Information Management Department at San Joaquin General Hospital to learn about documentation requirements to become a proxy.
